set it to always and every time phone turns off then back on it ask to grant access

Once you grant the access to AccuTracking (select GRANT then "Always", then press OK), the phone should remember it. You might forget to press OK to save it? If you did, perhaps it's a firmware issue.

what do you mean by firm ware issue

That means it could be a issue of the phone itself.
